Job Details
Key Responsibilities:
- Perform comprehensive health assessments, including patient history, physical examination, and diagnostic testing.
- Diagnose and treat acute, episodic, and chronic illnesses, providing appropriate treatment plans.
- Prescribe medications and therapies within scope of practice and in collaboration with supervising physicians.
- Educate patients and families on health conditions, preventive care, and healthy lifestyle choices.
- Conduct follow-up visits and modify treatment plans based on patient progress.
- Collaborate with multidisciplinary healthcare teams to optimize patient care.
- Maintain accurate, timely, and thorough documentation in electronic health records.
- Perform minor procedures as necessary and as trained.
- Participate in quality improvement initiatives to enhance patient care and clinical operations.
Qualifications:
- Master’s degree from an accredited Nurse Practitioner or Physician Assistant program.
- Valid state licensure as a Nurse Practitioner or Physician Assistant.
- Board certification in a relevant specialty (e.g., family medicine, internal medicine, etc.).
- Minimum of [X] years of clinical experience in a relevant healthcare setting.
- Proficiency in electronic health record (EHR) systems.
- Strong communication and interpersonal skills, with the ability to work effectively in a team setting.
