East Bay Community Action Program (EBCAP) is seeking a part-time Registered Nurse Care Manager for our Transgender Whole Healthcare program to provide comprehensive screenings, assessments, care coordination services, disease education, and self-management support. Nursing care is performed in both office-based and community-based settings, depending on the needs of the patient. This position will be based out of our 6 John H. Chafee Blvd. location in Newport, RI.
The Nurse Care Manager will be integrated into the health center and TWHP to manage complex medical needs experienced by the LGBTQIA+ and Gender Diverse communities including the following:
1. Facilitate necessary support around gender affirming care referrals including prior authorizations, insurance denial management, pre- and post-treatment planning, and assisting patients with identifying financial assistance options, including, but limited to programs such as: GoodRx, 340B Pharmacy Program, Medicare Part B, manufacturer prescription cost reductions programs, etc.
2. Facilitate private educational experiences relevant to patient success such as self-administration of GAHT injections and gels, home monitoring of blood pressure or glucose readings, and safe use of non-medical gender affirmation supplies such as chest binders and gaffes.
3. Provide weekly case debriefings to the TWHP Director, Clinician, and CHW.
4. Assist with patient evaluation and coordination of clinical labs to obtain pre- and post-measures for all relevant labs for patients receiving GAHT, PrEP, or menstrual suppression.
5. Provide PrEP Navigation for any TWHP or primary care patients receiving PrEP from EBCAP providers.
The successful candidate must have a minimum of an Associates Degree in Nursing or equivalent; licensed as a Registered Nurse (RN) by the RI Department of Health. Two or more years experience in a community health setting, acute care, or emergency/urgent care working with primary care providers to coordinate care and disease management. One (1) or more years’ experience supporting LGBTQIA+ and Gender Diverse patients required.
