Job Summary: Provides skilled occupational therapy services in the center consistent with professional standards and policies. Participates in the development of programs necessary to achieve patient care objectives. This position is delegated the responsibility and accountability necessary to carry out assigned duties and adherence with Company mission, values, policies and procedures. Education required in an accredited Occupational Therapy program and minimum of a Bachelor’s Degree. Qualification as a licensed Occupational Therapist in the state(s) of practice.
DUTIES AND RESPONSIBILITIES:
Demonstrates Competency in the Following Areas:
Representative Duties: (including but not limited to)
- Completes all documentation required and in accordance with state and federal regulations and policy.
- Completes all billing required in accordance with state and federal regulations and policy.
- Performs patient screens- annuals and nursing requests to determine need for services and or recommendations to care plan.
- Provides updates on patient’s status, progress, discharge planning as well as any follow up needs or items to Director and rehab team as needed.
- Attends other facility requested meetings per request of Director and or Administrator requests and reports to team and or supervisor as needed.
- Serve as a liaison between rehab department and center staff, physicians as well as families presenting a courteous, compassionate and respectful manner at all times.
- Responsible to develop a therapy plan of care with the overall plan of care to include goals and discharge plans per CMS and care center guidelines.
- Utilize standardized assessments to validate the need for skilled OT services as well as provide a patient based care coordinated course of treatment.
- Documents within time frames, while including the following as needed: observations, patient’s response to treatment, treatment programs, degree of progress and notable changes in patient condition.
- Communicates and coordinates patient care with other members of the patient’s health care team.
- Works collaboratively with rehab team, center staff, and supervisor to improve areas of performance as well as development of programming.
- Participates in educational in-service programs directly related to rehab services as well as center regulatory education required for compliance.
- Supervises occupational therapy assistants in accordance with regulations and policy.
- Displays competency in execution as well as application of various treatment modalities: Diathermy, E-stim, Ultra-sound.
- Accepts feedback and direction in a constructive and positive manner
- Provide patient and family teaching related to patient’s occupational therapy program ad its maintenance
- Ensures work areas are keeps clean, neat and safe environment
- Provides occupational therapy in accordance with physician’s orders and accepted professional practices.
- Attends and participates in workshops, seminars, etc. to keep abreast of current changes in the industry as well as to maintain a professional status.
- Maintain confidentiality of all pertinent patient/resident care information to ensure that patient/resident’s rights are protected.
- Demonstrates the ability to anticipate customer needs and to respond effectively and efficiently.
- Utilizes appropriate hand washing, standard precautions and personal protective equipment.
- Assist patient and / or family to procure needed equipment an or services for DC planning as requested by Director.
- Able to perform job responsibilities in a timely and efficient manner as directed and scheduled by Director.

- Maintains regulatory requirements, including all federal, state, local regulations and accrediting organization standards.

- Attends annual review and performs departmental in-services.
- Works at maintaining a good rapport and a cooperative working relationship with, departments and staff.
- Represents the organization in a positive and professional manner.
- Attends committee, CQI and management meetings, as appropriate.
- Resolves staff concerns at the departmental level, utilizing the grievance process as required.
- Ensures compliance with policies and procedures regarding department operations, fire safety, emergency management and infection prevention and control.
- Effectively and consistently communicates administrative directive to staff and encourages interactive departmental meetings and discussions.
- Complies with all organizational policies regarding ethical business practices.
- Communicates the mission, ethics and goals of the facility, as well as the focus statement of the department.
- Promotes a positive work environment and supportive of co-workers
