Occupational Therapist in the SNF setting - Colonial Poplin

Colonial Poplin Nursing Home in Fremont, NH is looking for a full-time Occupational Therapist. The Director of Rehab has been an employee of Colonial Poplin for almost 20 years and has a great team of PTs, PTAs, OTs, OTAs and STs. She collaborates with the Rehab Team and works alongside the Rehab staff as a PTA to provide care to our skilled, long-term and assisted living residents. We are looking for a motivated, energentic person to join our team. 

The position is a Full-Time, Monday-Friday position. No weekends required. Hours are negotiable based on employee needs. 

OT ESSENTIAL DUTIES AND RESPONSIBILITIES 

• Conducts resident screens (without interpretation) as allowed by state practice act.

• Helps residents with rehabilitative activities and exercises outlined in a treatment plan developed in collaboration.

• Monitors an individual’s activities to make sure they are performed correctly and to provide encouragement.

• Records resident’s progress.

• Provides training to residents, caregivers and families according to the resident’s care plan.

• Completes required forms and documents in accordance with company policy and state and/or federal regulations.
