Hours: 5 PM - 9 AM
Salary: $100K per year, plus $15,000 signing bonus
Our client, a large health care system in NYC, is seeking an On Call Center Services Registered Nurse Supervisor. The On Call Services RN Supervisor is responsible for the supervision of both the RNs (Triage & Field) and the CSRs who are emergently triaging the calls received from patients and/or caretakers for both Hospice, Palliative Care, and Home Care. This working supervisory role provides supervision of the staff that patient physical and
psychosocial health systems assessments telephonically, supporting the Hospice Plan of Care and Goals of Care based on the patients and caretakers needs/wishes, and facilitates coordination with plan developed/placed by the primary RN Case Manager for the patient.

Responsibilities:
- Reviews Hospice call center weekday and weekend requests for scheduled & projected on call visits.
- Prioritizes visit needs based on patient acuity and complexity.
- Assures assigned patient phone/visits are placed on clinician schedule accurately and timely based on schedule.
- Appropriately utilizes per diem staff to ensure patient coverage PRN.
- Builds and maintains positive relationships with all internal and external customers.
- Supports Call Center staff with clinical and non-clinical issues while following agency policy & procedure.
- Guides staff with all level of care changes including admissions, transfers to IPU, and Continuous Care assuring communication and coordination of care is maintained.
- Includes transfer of clinical record information appropriately.
- Documents level of care Transfer in Netsmart Assessment, Problems, and Clinical Notes as per policy.
- Contacts On Call Team Director or Administrator On-Call for clarification and direction as needed.
- Notifies On Call Team Director or AOC with unresolved issues, unresolved complaints, complex clinical issues, patient emergencies, and weather/agency emergencies PRN.
- Graduate from an accredited School of Nursing; BSN preferred.
- A minimum of two years experience in either Hospice, Palliative, Critical or End-of-Life care preferred.
- CHPN after one year of employment.
- New York State RN license, CPR-BLS and valid NYS Driver's license.
- Microsoft Office, Word and Excel.
- Ability to effectively direct and mentor the staff.
- Ability to constructively critique staff performance with the desire to mentor excellence in performance for the delivery of care.
- Ability to be dependable and capable of using initiative and good judgment.
- Possess effective communication, listening, and organizational skills.
- Possess strong clinical skills regarding end of life care.
- Ability to input clinical data proficiently.
- Ability to effectively communicate in a customer service environment.
- Ability to work flexible hours to meet the needs of the position.
- Ability to coordinate the resources and personnel involved in a
comprehensive interdisciplinary team environment. - Ability to work non-judgmentally with patient / family members of any culture, religion, socio-economic background or lifestyle.
- Detail oriented and ability to set own priorities.
- Ability to handle confidential aspects of assignments.
