Outreach Coordinator

Under the direction of the administrative staff, the Outreach Coordinator works as part of the Mobile Health team to provide confidential, non-judgmental harm reduction-based care. The Outreach Coordinator will use the Harm Reduction approach to help people who are marginalized get connected to medical care and obtain related support services.

The Outreach Coordinator serves as the Liaison for WFHC and acts as an advocate of WFHC by attending and engaging in community meetings, and social events. The primary responsibility of this role is to plan, organize, implement, and engage in community weekly, monthly, and annual events inclusive of Healthcare through Mobile Office, Health Expo/Fairs, Health Campaigns, and Weekly/Monthly/Annual events inclusive of: Parades, HIV National Testing Day, Juneteenth, Breast Cancer Awareness, Mental Health Awareness Month, etc., In addition, the HCEOC will identify community needs, develop wellness programs and initiatives, and track progress through WFHC electronic health systems.

Communication

Good oral and written communication skills are required for this position. Information must be exchanged using tact and persuasion appropriately. The person in this position must be able to communicate with the patients, staff, and others; and contribute to a positive environment. The ability to communicate in Spanish, Creole, or both is a plus.

Key Responsibilities

• Create and maintain a yearly calendar of outreach activities, including community events, workshops, appearances, and other communication opportunities inclusive of:
• Work closely with the Director of Administration, Manager of Social Services, Marketing, and Development Coordinator, and Quality Assurance Nurse Manager to coordinate outreach activities related to community and organizational health fairs, back-to-school, test and treatment for HIV, STI, and/or COVID, and events that are scheduled throughout the calendar year.
• Develop and maintain strong ties to the community and other service providers. Maintain a close relationship with key-partners within SLC and IRC.
• Coordinate with other outreach teams in joint outreach efforts.
• Drives and maintains Mobile Medical Office in accordance with state laws and company policies and procedures.
• Provides clinical coverage in the office as needed when work load allows.

Medical

• Provide health screenings, health education, and lab services including phlebotomy-based specimen collection, urine collection, and participant self-collected specimen processing. Package and store all specimens, per ACHAs guidelines.

Other

• Drive the mobile health van to perform essential duties.
• Enter appropriate data into the WFHC reporting system.
• Attend training as required to sustain certifications as it relates to testing and reporting.

Requirements

• Licensed Practical Nurse
• Experience with blood draws or a willingness to be trained to draw blood
• Strong written and oral communication skills
• Valid Driver’s License and access to reliable transportation.
• Must be able to operate a mobile health van to perform the essential functions of the position.

Working Conditions

Evening and weekend availability is occasionally required (8 hours shift, Monday-Friday, evening and weekends as needed).

Position schedule dependent on needs of participants and programs.

Ability to work outdoors within the community.
