PACE Center APP - Reid Health

We are a provider led organization providing excellent care for eight counties in Indiana and Ohio. Seeking a patient-centered and empathetic Advance Practice Provider to join our PACE Center. This individual will provide primary medical management in collaboration with a physician.

Position Details:

• Functions as a member of the PACE interdisciplinary team for collaborative decision making for the participants' plan of care.
• Spend at least 60% of the time working on coordination of care for each participant that includes detailed chart reviews, appropriate ordering of services, and identifying barriers that may lead to over utilization of services. 
• Team: RNs, LPNs, MAs, Physical Therapy, Occupational Therapy, Activities Coordinator, Social Worker, Community Liaison
• PACE Center open M-F, 8 a - 5 p
