Job Summary:
The Occupational Therapist (OT) will provide comprehensive assessments, interventions, and follow-up care for participants in the PACE (Program of All-Inclusive Care for the Elderly) Program. The OT will work collaboratively with an interdisciplinary team to enhance the functional abilities and overall quality of life for older adults, focusing on promoting independence in daily activities.
Key Responsibilities:
· Assessment and Evaluation:
o Conduct thorough occupational therapy assessments to evaluate participants’ physical, cognitive, and emotional needs.
o Develop individualized treatment plans based on assessment findings and participant goals.
· Intervention:
o Implement therapeutic interventions to improve participants’ functional abilities, mobility, and daily living skills.
o Provide education and training to participants and their families on adaptive techniques and equipment.
· Collaboration:
o Work closely with physicians, nursing staff, social workers, and other team members to coordinate care and achieve holistic outcomes for participants.
o Attend interdisciplinary team meetings and contribute to care planning discussions.
o Assist with Acute and Subacute stays and discharge planning.
· Documentation and Compliance:
o Maintain accurate and timely documentation of assessments, treatment plans, progress notes, and discharge summaries.
o Ensure compliance with all regulatory standards and policies governing occupational therapy practice.
· Community Engagement:
o Participate in community outreach and education programs to promote awareness of occupational therapy services and the PACE Program.
o Foster relationships with community resources to enhance participant support.
Qualifications:
· Education:
o Master’s degree or higher in Occupational Therapy from an accredited program.
· Licensure:
o Current state licensure as an Occupational Therapist.
· Experience:
o Previous experience working with older adults, preferably in a home care, rehabilitation, or PACE setting.
o Knowledge of geriatric care principles and practices.
Skills:
· Strong assessment and problem-solving skills.
· Excellent communication and interpersonal skills.
· Ability to work effectively as part of a multidisciplinary team.
· Physical Requirements:
· Ability to perform physical tasks associated with occupational therapy, including lifting, bending, and assisting participants with mobility.
· Work Environment:
· Work in various settings, including outpatient clinics, participant homes, and community-based facilities.
