Position Overview: The Pathway of Hope case manager effectively manages a caseload of families who seek social services assistance. This worker operates under a strength-based approach to interview applicants, determine eligibility, develop case plans for self sufficiency including goals and or budget forms, and maintains a trusting relationship with clients. The case manager provides social service assistance to eligible clients and serves as an advocate for the client in order to acquire services that would enable the client to achieve their goals
Responsibilities:Description of Position:
- Case Managers will be responsible for the provision of the full range of case management interventions including engagement of new clients, completion of comprehensive assessments and action plans, referrals to services designed to address identified goals and development of plans to address transition needs with ongoing documentation of the progress.
- Case Manager will be expected to complete all required client documentation on Service Point in a timely fashion. They will be expected to maintain case files for each client that includes all assessments, action plans, case notes, graduation plans, etc. They will ensure the quality of local data and reporting of information that will support National and Territorial Pathway of Hope program outcomes.
- Case Manager will meet with the divisional regional coordinator for individual consultation/ supervision to assist with the management of his or her caseload. Case Manager will participate in cluster meetings led by the regional coordinator.
- Case Manager will participate in local meetings with the Family Services Coordinator and Family Services Staff.
- Case worker will participate in Divisional or Territorial education forums and webinars designed to facilitate their understanding for a wide range of topics including motivational interviewing, cultural competency, strength-based case management, stages of change model, smart goals, time management and advocacy.
- Case Manager will maintain working relationships with community agencies to provide comprehensive services for participants and to stay abreast of current trends and resources available in the community. Case Manager will communicate with the agencies to get referrals of possible clients for Pathway of Hope.
- Case Manager will facilitate Financial Literacy Training Completion for Pathway of Hope Families.
- Case Manager will help with the planning & execution of Family Events for the POH Families.
- Case Manager will work with the Pastoral Care Representative to bridge spiritual resources and connect families with the Corps.
- Case Manager will keep clients informed of all corps programs & events.
- Case Manager will become acquainted with the Coats for Kids Program and Christmas Toy Shop events.
- Case Manager will help to keep track of Coats and Toys inventory.
- Case Manager will assist with the recruitment of children for Camp Ladore & the process for sending children to camp.
- Other duties as assigned.
Networking
- As directed, serve as a liaison on behalf of the Salvation Army Lancaster Corps with community groups in related fields. Work with community partnerships and inter-agency coordination meetings, coalitions, committees, and task force as appropriate.
- Maintain awareness of services provided by other local social service agencies, actively seeking resources for clients through these agencies.
- Cooperate with efforts of other community agencies to coordinate services and to communicate about difficult cases, assuring that valid client information releases are on file when appropriate and necessary
- Support the Lancaster Corps efforts to approach clients from the Salvation Army's holistic approach put forth in the local cord vision statement.
- Participate in internal committees and conferences as requested by the Corps Officer.
Recordkeeping and reports
- Maintain files on all clients interviewed/serviced including a description of the presenting problem, workers assessment of client needs, and record of assistance given or action taken as well as referrals made to other resources
- Ensure strict standards of confidentiality are met or exceeded.
- Maintain statistical information on services provided and outcomes achieved. Analyze data for the recommendation and implementation of program improvement measures.
- Enter client information into Service Point, in a timely manner.
- Complete paperwork and fund reports in an accurate and timely manner.
- Provide regional coordinator with information needed for monthly or quarterly reports.
- Present reports to Family Services Coordinator, Advisory Board or Corps Council as requested.
- Report any problems or inconsistencies concerning records, statistics and reports to the Corps Family Service Coordinator and Regional Coordinator.
- Perform all other duties as assigned.
Requirements:
- Bachelor of Social Work Degree, Master's degree preferred.
- A minimum of 3 years of experience in case management.
- Proven and ability to lead the clients in financial, spiritual, and life coaching
- Bilingual, Spanish preferred.
- Meets the Child Protection Clearances required by PA Act 153 to include PA Child Abuse History Clearance, PA State Police Criminal Record Check and FBI Criminal Record Check.
- As a Mandated Reporter, must provide proof of having completed training on recognizing and reporting child abuse meeting the training standards of PA Act 153 of 2014 within 30 days of employment with recurrent training required every five years thereafter.
- Must have an understanding of, and appreciation for the Mission of The Salvation Army.
