As a Patient Care Coordinator, you will be responsible for guiding patients through the health care system and ensuring those patients and their caregivers are able to access culturally and linguistically appropriate services in a timely and cost-effective manner. This position is responsible for the complex coordination of care Oncology patents require across multiple departments, ancillary, and UCM Network areas. Hybrid position that requires on-site support for Hyde Park main campus and UCM Oncology Network sites.
Essential Job Functions
- After initial new oncology appointment is completed serves as a single point of contact for return Oncology patients and is responsible to schedule appointments and coordinate referrals for testing, imaging, procedures, social work, physical therapy, occupational therapy, chaplain or spiritual services, geriatric-oncology, psycho-oncology, nutrition, palliative care, pharmacy clinic, SOCARE clinic, labs, clinical trials, and other care and support services at the hospital or in the community; contacts patients via phone, email, and/or letter to schedule, confirm, and send out reminders regarding appointments.
- Obtains prior authorizations, medical records, slides, specimens, faxes, and other items necessary for coordination of care; follows up with outside labs and other ancillary services for testing results and medical records and shares results with appropriate services as needed, including submitting results to Medical Records and patient’s community physician(s).
- Provides general education to patients and their caregivers about the care plan; Follows up with patients at regular intervals as defined to ensure that patients arrive to scheduled appointments on time and prepared. Fields patient and caregiver calls and questions related to appointments, ancillary services, or other matters related to coordination of care.
- Frequently reviews clinic schedules and templates and scrubs templates to optimize schedule for the day and ensure any rescheduling has been completed in advance.
- Provides patient navigation and care coordination across UCM Network Oncology Sites
- Performs other related duties as assigned.
Qualifications
- High School diploma, Bachelor’s Degree preferred
- Minimum of two years’ experience working in a high-volume complex healthcare environment
- Excellent telephone and personal communication skills are required as is proficiency with computer systems and programs
- Ability to multi-task and prioritize is crucial to success in this position
- Compassionate listening skills are required for patient and family distress related to new diagnosis
- Ideal candidate has strong interest and desire for extensive patient and family interaction
- Requires experience with and high commitment to customer service
- Uses discretion and good judgment, and maintains patient confidentiality
- Must be able to independently and professionally interact with clinicians and personnel, internal and external to UCM
