As a Patient Engagement Specialist at Horizon, you will… + Handle referral intake calls for all of Horizon Health Services Outpatient facilities
+ Provide excellent customer service, triage intake calls
+ Schedule outpatient appointments for all Horizon Health Services locations
+ Verify eligibility and benefits through EPACES, P-verify and HEALTHeNET
+ Provide phone support/resources to patients and families
+ Work with referral sources to gather information and provide linkage to Horizon Health Services

Here are a few specifics of what you’ll need for the job: + Bachelor’s degree in Human Services or related field required
+ One year of human services experience preferred
+ One year of clinical experience preferred
Location: 55 Dodge Road, Getzville, NY 14068 Hours: This position is full-time, Monday – Friday from 7:30am-4:00pm
Compensation Horizon is consistently evaluating our compensation and benefit packages to ensure we remain competitive
The range of pay for this position is $19.50 - $21.00
Starting pay may be impacted based on experience and education
Compensation adjustments are evaluated annually
