Responsibilities:
The Patient Representative plays an integral role in improving the patient/family experience while on hospice service. The Patient Representative provides supportive calls to the patients and families at end of life, maintains patient experience data and communicates patient/family needs and concerns to the teams and Quality Review Coordinator.
- Manages coordination of calls as initiated by workflow and as initiated by primary members of the IDT, noting need to follow up with phone calls and visits.
- Participates and contributes to the Interdisciplinary Team Process.
- Mandarin/Cantonese preferred.
- Displays professionalism during all communication with patients, families and co-workers.
- Reports all Patient Experience Program shift occurrences and complaints.
High School Diploma or equivalent required
Prior experience working within a healthcare environment, hospice preferred knowledge of hospice HIS requirements
Excellent verbal and written communication skills
- Use of various computer programs including Microsoft Word, Excel, and Power Point
