• Assists in the development and implementation of the interdisciplinary client care plan as it pertains to Physical Therapy.
• Treats clients to relieve pain, develop or restore functional, and maintain maximum performance, using physical means, such as exercise, massage, heat, light, and electricity.
• Teaches, supervises, and counsels the family and client in the physical therapy program.
• Instructs client in the care and use of adaptive devices such as: wheelchairs, braces, crutches, canes, and prosthetic and orthotic devices.
• Arranges for outpatient service for procedures when appropriate.
• Records on a timely basis all treatments, and client's response to therapy intervention Also records quality assurance data.
• Communicates with PT and other team members (including Home Health Aides), when appropriate. Instructs them in certain phases of physical therapy that they may use while working with the client.
• Under the direction of a Physical Therapist, writes clinical/progress notes on the date of the visit and incorporates same in the client's record according to company policy.
• Participates in case conferences, physical therapy staff meetings, agency activities, and in-service programs as necessary.
• Participates in effective coordination of schedule to ensure all reassessments are performed in a timely manner.
• Obtains a medical history, particularly as it relates to the present condition.
• Under the supervision of the physical therapist, assists in the evaluation and development of a rehabilitative plan of care and periodic re-evaluation as required.
• As directed, performs routine strengthening and therapeutic exercises and assists the patient with the use of self-help devices.
• Identifies possible ineffective drug therapy or adverse reactions, significant side effects, drug allergies, and contra-indicated medication and reports same to the PT.
• Observes, and record activities and findings in the clinical record and report to the PT and the physician any adverse reaction to treatment.
• Participates with staff, clients, and physicians in discharge activities and assists the Physical Therapist in completing the Physical Therapy Discharge Summary.
• Assumes other duties deemed appropriate and necessary by the PT and as designated by Supervisor.
• Awareness of the Agency’s QAPI program. Active participation as required per policies and procedures related to the Agency’s QAPI program.
• Management and follow-up of customer complaints, following Agency’s complaint policy
• Acute awareness of patient rights and the Agency’s responsibility in honoring patient’s rights. Awareness of patient rights description in Agency’s service agreement and Agency’s admission packet.
• Awareness of appropriate reasons for a patient discharge and Agency’s policy on appropriate reasons for discharge.
• Awareness of potential signs of abuse and neglect and the steps required to report suspected abuse and neglect.
• Awareness and ability to design and identify patient-centered and individualized goals with realistic measurable timeframes under the supervision of the Physical Therapist.
• Awareness of infection prevention and control techniques, both within their scope of practice and within the Agency’s policy and procedures. Awareness and participation in the Agency’s infection control program.
• Appropriate bag technique policy acknowledgment and demonstrated competence when applicable.
• Physical therapy equipment and other medical equipment will be utilized. Hand washing materials and PPE.

• The ability to drive and the ability to access clients’ homes, which may not be routinely wheelchair accessible are required.
• Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the client's condition and to perform client care.
• May be required to bend, stoop, reach, and move client weight up to 250 pounds, lift and/or carry up to 30 pounds.
• Must be a graduate of an approved school giving a course of not less than two years for Physical Therapist Assistants.
• Must be licensed by the State as a Physical Therapist Assistant.
• Must have one year of experience in an acute/sub-acute setting.
