Position: Home Health Physical Therapist

Overview: As a Physical Therapist with our Award Winning Client, you will deliver comprehensive Physical Therapy services within patients' homes. This role involves assessing patient functionality, developing tailored treatment plans, and improving patients' independence and physical/cognitive functioning. You will also educate patients and caregivers, supervise therapy assistants and certified nursing assistants as needed, and prepare detailed reports while engaging in agency development activities. Your role will require a Master's degree or higher and a valid Physical Therapist license in Illinois (IL), as well as a valid Driver’s License (DL). Preferred qualifications include experience in Home Health, and familiarity with OASIS & HCHB, which will be covered during orientation.

Key Responsibilities:

• Conduct assessments and design effective treatment programs
• Monitor and document patient responses and changes in condition
• Educate patients and caregivers on therapy techniques and device usage
• Oversee physical therapy assistants and certified nursing assistants
• Compile reports and participate in agency development activities

Schedule and Shift Details:

• Monday – Friday; day shift (start times based on patient needs); no patients after 5:00 pm
• 1 weekend every 6 weeks (Saturday & Sunday with scheduled patients) and 2 holidays per year

Travel:

• Local travel to patient residences

Qualifications:

Required:

• Master’s degree or higher
• Minimum of 1 year in a healthcare setting
• Valid Physical Therapist license in Illinois (IL)
• Valid Driver’s License (DL)

Preferred:

• Home Health experience
• OASIS & HCHB experience (preferred but will be provided during orientation)
• Please provide PT license number

Compensation:

• This role is a PPV position. Refer to attached PT PPV Rates for details, as rates vary based on service type and experience level.
• $10,000 bonus

Coverage Area:

• Gurnee, North Chicago to the East, Grayslake, the lake areas to the West, and the IL/WI border to the North
