PeaceHealth Home Health in Springfield, OR is seeking a Physical Therapist - Home Health for a Part Time, 0.50 FTE, Variable position. The salary range for this job opening at PeaceHealth is $44.25 – $66.37. The hiring rate is dependent upon several factors, including but not limited to education, training, work experience, terms of any applicable collective bargaining agreement, seniority, etc.
You will have the opportunity to work in the patient’s own environment engaging family and caregivers.
Our multi-disciplinary Home Health team consists of Physical Therapists, Occupational Therapists, Speech Language Pathologists, RN’s, Medical Social Workers, Registered Dietitians and Bath Aides. Our dynamic team meets regularly providing the opportunity to collaborate with supportive peers to coordinate care and achieve optimal outcomes.
Live, work and play in Eugene/Springfield, which is tucked in the lush Willamette Valley, just one hour from the coast and the mountains. We enjoy mild weather, and are the third largest metropolitan area in Oregon. Home of the University of Oregon, Eugene boasts cultural amenities typical of much larger cities, from the Jordan Schnitzer Museum of Art to the world-famous Oregon Bach Festival.
Job Summary
Responsible for the evaluation of patients, determining plan-of-care and goals of treatment, participating in treatment of patients within the scope of Physical Therapy. May provide oversight to therapy assistants and/or students in following the plan-of-care and treatment of patients appropriate to the age of the patient served; may provide supervision to therapy aides and/or volunteers. Works as part of a patient care team toward the best outcomes for the patient.
What you will do:
+ Evaluates assigned patients and establishes a treatment plan and goals, using data from a variety of sources, such as patient records, interviews, observation, and team members.
+ Provides patient treatment according to plan of care and modifies plan to meet patient needs, and updates plan as appropriate.
+ Completes documentation accurately, timely, according to regulatory and state licensure requirements, and within professional standards.
+ Makes recommendations for relative content and scope of therapy services and implements policies and procedures that guide and support the provision of services.
+ Recommends and develops new therapy services and programs in collaboration with staff.
+ May spend minimal time to assist with evaluating and orienting new caregivers and making recommendations to management regarding equipment and space needs related to physical therapy functions.
+ Collaborates with other healthcare professionals as necessary to ensure smooth continuous care of the patients.
+ May act as a liaison with nursing, medical and other therapy staffs to facilitate problem solving and coordination of other services and act as an educational resource.
+ May mentor and provide oversight to new caregivers, physical therapist assistants, and/or therapy aides. Provides clinical instruction and training to students.
+ Recommends and participates in development of new services and programs, quality improvement and monitoring activities.
+ May participate in staff meetings, patient care meetings, educational in-services, caregiver orientation and other professional activities.
What you bring:
+ Bachelor's, Master's or Doctorate Degree Required: Physical Therapy accredited by the Commission on Accreditation in Physical Therapy Education or closely related field (foreign equivalent accepted)
+ Required : Physical Therapist license State of Oregon
+ Required Upon Hire: Basic Life Support through American Heart Association or American Red Cross
+ Valid driver’s license and proof of auto insurance.
+ Requires one year of Physical Therapist experience, with home health/geriatric experience preferred.
