About the Job
Essential Job Responsibilities
1. Performs evaluations, develops patient specific goals, and implements treatment programs.
2. Evaluates the effects of treatment, assesses progress, provides medically necessary modifications to ongoing treatment plans.
3. Documents patient care services timely and according to accepted regulatory, corporate, and professional guidelines.
4. Supervise Physical Therapy Assistants per regulatory guidelines.
5. Communicate and document the need for supplies and adaptive equipment for patients to facilitate independence. Instructs, trains, and documents the use of recommended equipment.
6. Instruct patients, families, and/or caregivers in maintenance programs.
7. Refers to other disciplines to provide skilled services for functional deficits or implementation/revision of restorative or maintenance programs.
8. Recommends continued community follow-up programs and equipment to facilitate the highest level of independence.
9. Maintains patient care standards for the care of a patient with a tracheostomy, ventilator, and/or g-tube.
10. Participates and contributes to patient care meetings, to advocate and provide the appropriate care for the patient. Attend other related meetings
11. Understanding of post acute facility regulations and guidelines, for various governing bodies. Including but not limited to company compliance program and code of conduct, and HIPAA.
12. Promotes a safe operating environment to eliminate work place hazards.
13. Ensures proper operation of equipment by complying with company procedures and following manufacturer’s instructions/guidelines.
14. Performs other related duties as required.

Minimum Qualification Requirements
1. Current, valid, in good standing license as a Physical Therapist in the State of Hawaii. Must maintain an active license in good standing throughout employment.
2. Graduate from a CAPTE accredited curriculum in Physical Therapy.
3. Experience working with a variety of diagnoses and equipment. Including but not limited to, electrical stimulation, ultrasound, and alternative augmentative communication devices.
4. Valid driver’s license in the State of Hawaii.
5. Knowledge of skilled nursing home regulations, procedures, laws, regulations, and guidelines pertaining to long-term care.
6. Maintain a current BLS CPR certification.
7. Ability to effectively read, write, speak, and comprehend English.
Preferred Qualifications
1. Tracheostomy and Ventilatory experience.
2. Passy Muir Valve experience.
Shift Information
1. PART TIME shifts may include weekdays, weekends, and occasional weekdays.
