Job Description

The Physical Therapist reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.

The Physical Therapist / Athletic Trainer, in collaboration with the physician, provides nutritional counseling to restore or maintain an optimal nutrition status for the patient. This position communicates and collaborates effectively with the health care team to exchange and disseminate information regarding care of the patient and program development.

Description

· Collaborates with the physician to set objectives for the patient based on medical history, physical limitations, evaluation of cardiovascular risk factors and current fitness level.

· Meets with patients to assess motivation and stage of readiness to change.

· Identifies appropriate physical activity, exercise, and behavioral intervention.

· Develops and implements exerciseplan/program.

· Conducts hands-on training, including demonstrating/correcting proper exercise form, and leadingindividuals through a customized workout.

· Educates on the role of exercise in chronic disease management (including,but not limited to, obesity, cardiovascular and pulmonary disease, hypertension, diabetes, osteoporosis, cancer anddepression)

· Provides on-going consultation and evaluate effectiveness of intervention.

· Develops, presents, and updates educational materials to promote education and behavioral goals for the patient (including developing, maintaining, andcontinuously updating a library of written materials, wellness newsletters, online programs, and apps about exercise).

· Identifies and communicates key issues and trends to the health care team and participate in in-service training for staff.

· This role may support additional projects, perform administrative tasks, and other duties as assigned.

· Consults with other programs/staff to ensure coordinated service delivery.

· Identifies and communicates key issues and trends to health care team.

· Participates in in-service training for staff, students, and physicians.

· Educates target groups about the role of exercise management and lifestyle medicine.

· Collects and documents research data to support quality management outcomes.

· This role may support additional projects, perform administrative tasks, and other duties asassigned.

Qualifications

Required:

• Physical Therapy, Licensed (IL).
• Basic Life Support (American Heart Association) certification.
• Master of Science or Doctorate in Physical Therapy (if graduated after 2002).
• Bachelor of Science Degree in Physical Therapy (if graduated before 2002).
• Zero (0) to two (2) years experience in a relevant role.
