The Physical Therapist is responsible for providing a full range of Physical Therapy services including assessment, treatment planning and therapeutic interventions in an interdisciplinary environment consistent with the position's qualifications, professional practices and ethical standards.
PRIMARY RESPONSIBILITIES
- Establishes and evaluates an appropriate plan of care with patient/family related to patient deficits/diagnoses.
- Performs therapy interventions utilizing standard physical therapy techniques and skills as appropriate for the condition of the patient.
- Provides education to patient/family /staff by assessing patient/family educational needs.
- Helps implement continuity of care by participating in discharge planning to ensure patient receives adequate continued treatment upon discharge.
- Maintains appropriate documentation that details the patient's response to therapy and the need for skilled intervention as outlined by regulatory bodies.
- Evaluation of patient based on physician order as well as the evaluation of ongoing and final effectiveness of treatment.
- Provides age-appropriate modalities and therapeutics interventions directly to patient as prescribed by physician, and in accordance with established policies and procedures.
- Provides educational assistance to patients, families, students, other hospital personnel and to the general public.
- Communicate and demonstrate a professional image/attitude for patients, families, clients, coworkers and others
- Participates in the performance improvement activities of the unit or department.
- Reads and initials all department memos and meeting minutes.
- Attends all mandatory Hospital-wide Education seminars.
- Contributes to the delivery of cost-effective care by utilizing resources appropriately and maintaining equipment in order to prolong working life
- Performs all other duties as assigned by Supervisor.
EDUCATION/EXPERIENCE
- High school graduate or equivalent; additional college training preferred
- 1 year healthcare experience preferred
EDUCATION AND TRAINING:
- Graduate of an APTA approved Physical Therapy Program.
LICENSING/REGISTRATION/CERTIFICATION:
- Holds current state licensure or eligible for permanent license within 6 months of hire. CPR certification.
EXPERIENCE:
- Experience in working with an adult and geriatric population. Education/experience with rehabilitation and / or a physically challenged population is preferred.
- SKILLS AND ABILITIES:
- Ability to lift/transfer patients and equipment. Ability to perform frequent and prolonged bending, stooping, crouching, walking and standing. Ability to communicate effectively and empathetically with patients, families, and coworkers. Time management, clinical reasoning, screening and assessment skills emphasized. Use of personal computer and word processing applications.
