The complete scope of PT tasks will fall under the supervision and oversight of the Physical Therapist who will develop, implement, and assess all therapy-based initiatives, in alignment with individualized and group care plans. This role requires direct care as well as the need for strategic, analytical, and creative thinking and the ability to inspire others to excel. Core responsibilities include:
- Evaluating all members’ mobility levels and specific PT needs, devising comprehensive approaches to increasing strength, flexibility, ROM, motor control, neuromuscular reeducation and related pain reduction/discomfort, in keeping with new and existing medical and social needs
- Developing innovative treatment programs and setting goals for members and caregivers, then devising a periodic check-in plan/procedure to gauge progress
- Evaluating members’ home environments to minimize hazards and provide additional in-home support as needed
- Documenting all changes in care plans and therapeutic recommendations, as well as participation in individual therapies and group activities in Data Pace
- Assessing members’ functional status and adjusting/enhancing approaches as needed
- Identifying areas of potential improvement, for both individual members and the LIFE Center community as a whole, then develop and implement these positive changes accordingly
- Completing fall risk assessments and developing individual plans to reduce these vulnerabilities
As a core participants in members’ care, the Physical Therapist is expected to be an active stakeholder on interdisciplinary teams, working in tandem with physicians, nurses, dieticians, occupational therapists and other caregivers to provide meaningful updates, recommendations, timelines and other essential information that best steers ongoing Plans of Care. The ability to work well in a team environment, strong communication skills, and versatility are all essential traits Senior LIFE looks for in a Physical Therapist, as is a genuine desire to aid an elderly community.
All applicants must be licensed to practice physical therapy in Pennsylvania, plus a relevant degree from an accredited college or university and at least one year of experience working specifically with an elderly/geriatric population. Candidates will be expected to demonstrate strong clinical competency and the ability to troubleshoot diverse circumstances. A valid Pennsylvania driver’s license is also required.
Vaccination from covid-19 is a condition for hiring. A request for exemption can be submitted and will be reviewed and approved on an individual basis.
