About the Job
The Rainbow KASA program at Boston Children’s Hospital is an interdisciplinary program which provides primary care and care coordination services for medically complex patients at our three primary care locations. An interdisciplinary team of doctors, nurse practitioners, nurses, and social workers support families through inpatient discharge planning, complex care coordination services, population health management, and health-related social needs services.
Under direct supervision, the Population Health Assistant shall be responsible for:
- Coordinating patient scheduling, population reporting, and related support activities and functions for the Rainbow/KASA population, a group of medically complex children receiving primary care services.
- Providing administrative support for assigned project teams, including development of educational tools and training materials for quality improvement projects and data management related to assigned population
- Preparing monthly or quarterly reports on population activities progress, status, or other special reports, as assigned, to present to the clinical team.
- Drafting written communication and promotional literature as part of the Rainbow/KASA outreach efforts.
- Interacting with and maintaining liaison with staff, faculty, and outside/community agencies in facilitating program objectives.
- Providing administrative support for assigned project teams, including development of educational tools and training materials for quality improvement projects and data management related to assigned population.
- Providing general project coordination, including scheduling patients, meetings, agenda setting, note taking, and action item completion tracking
- Provides direct administrative support in obtaining documentation and other materials for patient care.
To qualify, you must have:
- High School Diploma/GED. Bachelor’s degree may substitute for one year of experience
- 1 year experience in customer service or clinical research assistant work.
- Strong verbal and written communication skills and active listening skills. Communicates clearly and appropriately.
- Basic computer navigation and keyboarding skills, including ability to multi-task and navigate between multiple systems.
- Basic skill in collecting and analyzing data.
- Strong and accurate data entry.
- Ability to work with confidential information in a professional manner.
- Basic knowledge of Microsoft Office Suite.
