The Program Manager will oversee Linkage team. They will provide direct supervision of team members, and manage employee workflow ensuring that individuals receive the correct level of care. The linkage team will screen clients applying for services and either provide short term case management or transition clients with higher acuity and higher needs to outreach teams. They will report on capacity, service needs, and trends to leadership. The Program Manager will provide and ensure the team provides strengths-based, client-centered, and trauma-informed assessments and services to clients with severe mental illness and co-occurring substance abuse/addiction disorders.
Responsibilities
- Manage linkage services to meet client and community needs at point of intake (i.e. expansion of case management services to other site locations, seek new funding opportunities to support community efforts to address behavioral health needs, etc.).
- Track agency capacity and assist in ensuring new referrals are connected with the appropriate level of care
- Manage communication regarding capacity in programs within the organization; and alert Director of excess capacity issues and programs that are above recommended capacity
- Develop and implement systems to increase department efficiency; i.e. reduce time on linkage, smooth transition of individuals from linkage to outreach teams, etc.
- Train new employees to provide quality services to clients, including New Staff Orientation facilitation and employee on-boarding.
- Ensure continued education and training for assigned clinical and administrative employees to strengthen services provided.
- Monitor and adjust employee schedules accordingly to ensure enough clinical time is scheduled to meet productivity requirements.
- Ensure employees review organizational policies and procedures; and understand duties, projects, and priorities of the department.
- Oversee state and grant contracts and develop systems to ensure utilization of funds
- Manage the implementation of special projects, grants, and new programming
- Ensure completion and submission of reporting requirements; and review/revise program manuals as needed.
- Ensure services are available for the highest need clients applying for service when ACT and CST teams are at capacity
- Provide on-call coverage as assigned.
- Perform other related duties and/or projects as assigned.
Qualifications
- Masters’ Degree in Social Work, Psychology, or Counseling required
- Licensed Clinical Social Worker, Licensed Clinical Professional Counselor, or Licensed Practitioner of the Healing Arts required
- Minimum three years’ experience working with people with mental health and/or substance abuse diagnoses
- Valid Illinois driver’s license with daily access to a well-maintained vehicle with $1,000,000/$3,000,000 liability insurance
- Previous supervisory experience and marketing skills preferred
