Program Specialist
- Duties: Provide companion support to individuals with intellectual disabilities in a community home setting, comply with chapter 6400 regulations, the department of human services standards, and Allegheny Community Home Care policies and procedures. Ensures the health, safety and welfare of the individuals supported. Reports to the ACHC Senior director.
- Responsibilities:
- Coordinating and completing assessments.
- Providing the assessment as required under 6400.181 (f) (relating to assessment)
- Participating in the development of the ISP, ISP annual update and ISP revision.
- Attending all ISP meetings.
- Fulfilling the role of plan lead, as applicable under 6400.182 and 6400.186 (f) & (g) (relating to developmental, annual update, and revision of the ISP; and ISP review and revision)
- Reviewing the ISP, annual updates, and revisions under 6400.186 for content accuracy.
- Reporting content discrepancy to the SC, as applicable and plan team members.
- Implementing the ISP written.
- Supervising, monitoring, and evaluating services provided to the individual.
- Reviewing, signing, and dating the monthly documentation of an individual's participation and progress towards outcomes.
- Reporting a change related to the individuals needs to the program manager and SC as applicable and plan team members.
- Reviewing the ISP with the individuals as required under 6400.186.
- Documenting the review of the ISP as required under 6400.186.
- Providing documentation of the ISP review to the SC as applicable, and plan team members as required under 6400.186 (d).
- Informing plan team members of the option to decline the ISP review documentation as required under 6400.186 (e).
- Recommending a revision to a service or outcome in the ISP as provided under 6400186 (e).
- Coordinating the service provided to the individual.
- Coordinating training of direct service workers in the content of health and safety needs relevant to each individual.
- Incident management point person.
- Maintain HRST for each individual under their care.
- Developing and implementing provider services under 6400.188 (relating to provider services)
- Supports the achievement of individuals personal outcomes.
- Implements individuals' community life plans.
- Engages individuals living within the house with leisure activities such as arts, crafts, and games.
- Provides general assistance to the individuals in the home with implementing their activities of daily living.
- Completes documentation as it relates to their job responsibilities.
- Complies with instructions given by the program manager in the event of a crisis situation.
- Serves as a model to the appropriate language, dress, and behavior.
- Receives 24-hours of training annually, relevant to human services.
- Assumes other responsibilities as required.
- Qualifications:
- Masters' degree from an accredited college or university and 1-year work experience work directly with persons with IDD,
- Bachelor's degree from an accredited college or university and 2 years work experience work directly with persons with IDD, or
