Summary
Works with a multidisciplinary team to conduct the Outpatient Cardiac Rehabilitation Program. Performs in a lead role in the provision of telemetry-monitored exercise sessions and coordinates appropriate follow-up with referral sources and the Medical Director as needed. Treatment and education is provided in the areas of deficit to assist patients in safely regaining functional abilities and adopting a heart-healthy lifestyle.

Education
Two years of formal training or education beyond the high school level (e.g., Associates Degree).
Certification, Registration & Licensure
Certification required: Current American Heart Association Basic Life Support (BLS) for Healthcare Providers certification, or equivalent course credentialed by American Heart Association.
Registration required: State of New Hampshire Registered Nurse.
Experience
None required. Must possess knowledge of growth and development across the life span. Ability to effectively and positively interact with a multi-disciplinary staff, patients and others while being a member of a team.
Responsibilities
Performs Cardiac Rehabilitation Program and Pulmonary GOLDĀ initial evaluations.
Provides group and individual education related to disease process, medications, risk factors, etc.
Manages patients in distress.
Flexible with work assignments.
Provides clinical monitoring for patients.
Management of insurance requirements.
Meets department productivity standard.
Documents departmental specific requirements.
Communicates with referral source or Medical Director.
Fills charge sheets out.
Participates in conducting Pulmonary Rehabilitation
