A healthcare organization New York City is currently seeking an experienced, licensed Registered Nurse (RN) to join their team as a Care Manager.
About the Opportunity:
- Setting: Managed Care
- Patient Population: Managed Long Term Care (MLTC)
- Hours: Monday through Friday, business hours
- Work Model: Role is hybrid during training and can be fully remote after training
Responsibilities:
The Care Manager (RN) will:
- Provide case management to members, including medical, social, psychological, and physical.
- Develop, implement, and monitor the care plan to help meet the member's needs
- Perform other duties, as needed
Qualifications:
- Strong MLTC experience either in UAS or case management experience
- Graduate of an accredited School of Nursing
- Strong computer skills needed
- NYS Registered Nurse (RN) license
Desired Skills:
- Bachelor's Degree in Nursing
- Bilingual is always a plus, but not required
