About the Job
Provides direct nursing care in accordance with established policies, procedures and protocols in assigned specialty.
The starting range for this position is $65,000- $80,000 annually.
Responsibilities:Patient care
- Participate in the coordination of patient care management as part of the care management team for high risk populations.
- Provide health-related education as directed by provider or as indicated by nurse assessment.
- Participate in collaboration with the health care team in identifying, evaluating, and prioritizing patient and family needs to develop a plan of care.
- Responsible for full compliance with all applicable regulations for the operation of a Class D Pharmacy.
- Telephone/walk-in triage of patients using approved national standards.
- Perform patient medication reconciliation in conjunction with the provider as part of the pre-visit assessments and care coordination, including coordinating refills with providers at time of visit.
- Follow up with patients as directed by provider.
- Perform Birth Control Refill visits and patient education as needed.
Staff management
- Refer to and resource other extended care team members (such as the Unit Clerks, Medical Assistants, Health Ed, Referrals or other resources) to maximize patient flow and provider productivity.
Ancillary functions:
- Participate in chart audits as per protocols to monitor quality of care.
- Route incoming patient records and forms as appropriate.
- Communicate home health referrals and notify providers.
Support team and Clinic functions:
- Participate in care team huddles and other staff meetings as required.
- Participate in problem identification and resolution in functions relevant to duties.
- Maintain effective communication with Unit staff, other floor personnel, Clinic management and all departments, acting as a resource within the clinic to other care team members.
- Demonstrate support for decision making, policies and procedures.
- Participate in quality improvement processes and projects, including policy and procedure development and updates.
- Identify opportunities to improve work flow, work processes and patient care, and work cooperatively to implement these and other quality improvements.
- Adhere to HIPAA guidelines.
Education: RN, graduate of an accredited school of nursing.
Experience: Two years’ related experience preferred.
Certificate/License:
- Current professional license to practice in the State of Texas.
- CPR Certified
Knowledge, Skills, and Abilities:
- Bilingual in English/Spanish
- Strong written and verbal communication skills, including reflexive listening;
- Skill in public relations and group presentations
- Basic counseling skills and health education knowledge, including understanding of human anatomy and physiology
- Able to establish and maintain cordial and professional relationships with patients, volunteers, and Clinic staff;
- Able to develop and use resource and referral systems.
- Able to work independently, exercising personal initiative.
