\nThe staff member must\ndemonstrate the knowledge and skills necessary to provide care appropriate to\nthe age of the patients served on his or her assigned unit. The individual must\ndemonstrate knowledge of the principles of growth and development of the life\nspan and possess the ability to assess data reflective of the patient’s status\nand interpret the appropriate information needed to identify each patient’s\nrequirements relative to his or her age-specific needs and to provide the care\nneeded by pediatric adolescent adult and geriatric patient group.
\nOccupational exposure to blood\nborne pathogens is reasonably anticipated. \nRequired tasks involve a potential contact with blood body fluids\ntissues.
\nAssess the biophysical and psychosocial needs of\nthe patient on admission and on an ongoing basis throughout the hospital stay\nensuring that the appropriate level of care is assigned and maintained and that\nthe appropriate discharge plan is in place. Through daily rounds on the patient\ncare units ensure that the right care is provided in the right place by the\nright provider at the right time and at the most appropriate cost. Collaborate with patient family physician\nand team members to develop a transition plan appropriate to the patients\nneeds and financial capabilities. \nConducts a readmission assessment on all readmissions within 30 days of\nindex admission discharge date.
\nIdentifies needed services and coordinates\nreferral to appropriate agency/service provider. Works as a health team member to ensure that services are\navailable and in place in a timely manner. Coordinates as needed a\nmultidisciplinary care planning process for patients meeting criteria and/or\nare referred by other team members. Facilitates implementation of clinical\npractice guidelines assists other professional disciplines in effectively\ndeveloping and integrating support modalities into guidelines to ensure that an\nefficient comprehensive plan of care is established. Provides ongoing\neducation to patients and families as it relates to care planning and\npreparation for discharge. Serves as a\npatient advocate. Enhances a\ncollaborative relationship to maximize the patients and familys ability\nto make informed decisions. Make daily\nrounds with assigned physicians based on priorities of patients needs as\navailable.
\nDocuments and reports quality issues risk management infection control\ncomplaints and efficiency issues in the provision of care interface with the\nPerformance Improvement Department in concurrent and retrospective monitoring\nof compliance with clinical guidelines. Completes\nthe patient and family caregiver interview tool on all readmissions within 30\ndays of index admission discharge date and forwards tool to Director Care\nManagement. Provides orientation and\nmentoring to new staff and students.
\nActs as a liaison to post hospital providers and\nother community health resources. Provides verbal and written communication\nwith physicians nursing and ancillary staff as necessary to ensure continuity\nof care. Has the ability to influence decisions gain cooperation and\nfacilitate change with physicians all levels of management and employees and\nsupervisors of other hospital departments. Is knowledgeable and follows\nregulatory requirements of reporting to Child Protective Services Adult\nProtective Services and other agencies. \nParticipates in interdisciplinary patient care rounds and/or conferences\nto review treatment goals optimize resource utilization provide family\neducation and identified post-hospital needs. \nDocuments clearly and accurately in the medical record in accordance\nwith departmental organizational and regulatory agency guidelines.
\nReviews for medical necessity utilizing\nInterQual Criteria. Demonstrates ability to identify barriers that impact\npatients’ length of stay and corroboratively develops and implements strategies\nto decrease length of stay. Monitors appropriateness of test/procedures\nconsultations treatment plans and resource utilization. Anticipates\nbarriers/variances to the delivery of care intervenes as necessary. \nInitiates and presents denial letters as appropriate. Issue Medicare adverse determination letters\nwhen deemed appropriate.
