**Job Title:** RN Case Manager **Job Specialty:** Case Management **Job Duration:** 8-12 weeks, Full time (40 hours per week). Open to part-time as well. **Shift:** 08:00 AM - 04:30 PM, Days (Monday - Sunday), Holiday rotation required, Weekends required **Guaranteed Hours:** Not specified **Experience:** Inpatient Acute Care setting **License:** Registered Nurse (RN) **Certifications:** Basic Life Support (BLS) **Must-Have:** Experience in case management and inpatient acute care **Job Description:** - Provide case management services to inpatient acute care patients. - Coordinate with medical staff, patients, and families to create effective care plans. - Ensure efficient utilization of resources and facilitate timely patient discharge. - Create and maintain accurate documentation of case management activities. - Participate in interdisciplinary team meetings to review and update patient care plans. - Comply with all healthcare regulations and standards. - Engage in continuous professional development activities. - Participate in holiday rotations and work as required on weekends.
