About The Position
**Job Title:** Care Manager **Job Specialty:** Emergency Department (ED) **Job Duration:** 9 months **Shift:** Day - 5x7.5, 9am - 5pm **Guaranteed Hours:** 40 hours per week **Experience:** Two to three years of clinical experience in a certified home health agency, Lombardi program, and/or managed long-term care or managed acute program. Five years minimum of clinical experience as a Registered Nurse. Leadership experience preferred. **License:** RN licensed as a Registered Nurse in the State of New York. **Certifications:** PRI and Screen certification required within three months of employment. Certification in Case Management preferred. **Must-Have:** - Bachelor’s degree required; BSN preferred - Experience with care coordination, utilization review, and discharge planning - Computer literacy - Bilingual in Spanish (Read, Write, Speak) **Job Description:** - Coordinate and manage patient care in a long-term care setting - Perform utilization review to ensure the efficient use of resources - Develop and implement discharge plans for patients transitioning from acute care to long-term care settings - Collaborate with various healthcare professionals to ensure comprehensive care - Maintain detailed and accurate patient records - Provide leadership and guidance to nursing staff involved in patient care coordination - Communicate effectively with patients and their families, particularly those who are Spanish-speaking.
