About The Position
**Job Title:** RN Case Manager - Home Health **Job Specialty:** Home Health Nursing **Job Duration:** 13 weeks **Shift:** Day - 5x8 - 8:00 AM - 4:00 PM, with every 4th weekend rotation **Guaranteed Hours:** 40 hours per week **Experience:** Minimum of one year of Medical/Surgical nursing experience required; home care/hospice experience preferred **License:** Current license to practice as a Registered Nurse **Certifications:** Current American Heart Association BLS certification **Must-Have:** - Graduate of an approved, accredited school of professional nursing; BSN preferred - Demonstrated excellent verbal and written communication skills - Strong interpersonal skills - Demonstrated organizational and time management skills - Current driver's license, a good driving record, and reliable transportation **Job Description:** - Provide skilled care and case management to patients and families - Operate in accordance with the State Nurse Practice Act and accepted professional standards - Utilize nursing theory and processes for services - Communicate and document observations and assessments - Promote and maintain health through direct care, teaching, counseling, and rehabilitative measures - Report to the Clinical Team Manager - Follow established organizational and departmental policies and procedures
