About The Position
**Job Title:** Registered Nurse **Job Specialty:** Discharge Planner **Job Duration:** 13 weeks **Shift:** Day 5x8-Hour (08:00 - 16:30) **Guaranteed Hours:** Not specified **Experience:** 2 years minimum experience required; must have 1 prior travel assignment **License:** Must have DC License in hand; must be eligible for LMSW/LGSW/LCSW if applicable **Certifications:** BLS, RN or LMSW/LGSW/LCSW **Must-Have:** - Experience with discharge planning - Obtaining facility choices - Sending referrals - Issuing IMM and other Medicare letters - Must have the Flu vaccine (no exemptions) **Job Description:** - Perform discharge planning and case management for various patient diagnoses including geriatrics, ICU, heart and vascular, general surgery, and general medicine. - Work in settings such as acute hospitals, insurance companies, long-term acute care/rehab, and skilled nursing facilities. - Utilize Cerner as a required work setting skill. - Engage in admission criteria, benefits eligibility, care coordination, discharge planning, needs assessment/order DME, plan of care, pre-cert review, prior authorizations, and worker's comp case management. - Adhere to regulatory skills like CMS, Department of Health, HIPAA, and The Joint Commission/Core Measure/National Safety Goals. - Preferred skills include experience in home health, hospice, telephonic case management, and previous charge experience. - Required to work on holidays as needed and every 5-6 weekends. - Follow a business casual dress code; appearance should be clean, neat, and professional. No scrubs. - Participate in orientation for 28 hours at %50 non-billable rate. - Complete billable modules up to 4 hours; average 12 hours on modules, which can vary by specialty. Modules are recorded via transcripts.
