About The Position
**Job Title:** Registered Nurse **Job Specialty:** Case Manager/Discharge Planner **Job Duration:** 13 weeks **Shift:** Day, 5x8-Hour (08:00 - 16:30) with rotating weekends **Guaranteed Hours:** **Experience:** Minimum 2 years required, must have 1 prior travel assignment **License:** Must have a valid DC License in hand (for DC locations) **Certifications:** BLS, RN, or LMSW/LGSW/LCSW **Must-Have:** Experience with discharge planning, obtaining facility choices, sending referrals, issuing IMM and other Medicare letters **Job Description:** - Serve as a Social Worker/Discharge Planner - Work 40 hours a week during the day shift from 8:00 a.m. to 4:30 p.m. - Be available to work every 5-6 weekends for one shift as needed - Possess skills in Cerner, Acute Hospital, Insurance Company, and Long Term Acute Care/Rehab/Skilled Nursing - Preferred experience in Home Health, Hospice, telephonic, and previous charge experience - Demonstrate skills in Case Management/Utilization Review including Admission Criteria, Benefits Eligibility, Care Coordination, Discharge Planning, Needs Assessment/Order DME, Plan of Care, Pre-Cert Review, Prior Authorizations, and Workers Comp Case Management - Possess required regulatory skills in CMS (Centers for Medicare and Medicaid Services), Department of Health, HIPAA (The Health Insurance Portability and Accountability Act), and The Joint Commission/Core Measure/National Safety Goals - Preferred regulatory skills in Workers Compensation - Manage patient diagnoses including Geriatrics, ICU, Heart and Vascular, General Surgery, General Medicine, and MICU - Adhere to a business casual dress code ensuring a clean, neat, and professional appearance (no scrubs) - Mandatory flu vaccine compliance with no exemptions allowed - Complete orientation of 28 hours at 50% non-billable Please note that the position requires a commitment to not request time off during the first two weeks of the assignment.
