About the Role:

We're seeking an experienced Registered Nurse to join a dedicated home health team, managing and delivering patient care in Carnegie/Crafton, PA, and surrounding areas. This role requires a detail-oriented RN to manage patient care plans, maintain critical documentation, and provide weekend on-call support.

Your Mission:

• Manage & Deliver Care Plans: Collaborate to develop and execute patient care plans tailored to individual needs.
• Patient Support & Travel: Provide in-home care to patients, spending about 20% of your day traveling between locations.
• Documentation & Deadlines: Complete charting promptly to meet critical documentation deadlines.
• Self-Management: Effectively manage time and scheduling in a mobile role with patient-centered focus.

Core Competencies:

• Proficiency in clinical documentation, informatics, or quality management.
• Strong ability to use EMR systems with experience in Practice Partner EMR or similar.
• Compassionate care, attention to detail, and effective communication with patients and families.

What You Bring:

• Education: Associate degree or higher in Nursing.
• Experience: Minimum of 1 year in an RN role, with experience in clinical documentation or quality preferred.
• Licensure: Active Registered Nurse (RN) license in Pennsylvania.
• Certifications: ARC ALS, AHA ACLS, or both.
• Driver's License: Valid driver's license with reliable transportation and auto insurance.

Schedule & Shift Details:

• On-Call Schedule: Friday-Sunday from 5:00 PM to 10:00 PM, plus 8 units on Saturday and Sunday.
• Holiday Requirement: 2 holidays per year (1 summer, 1 winter).
• Travel: Local travel within Carnegie/Crafton and surrounding areas.

Compensation & Benefits:

• Pay Structure: Pay-Per-Visit rates ($42-46 per visit) with a guaranteed $3,000 stipend per pay period during the 6-month orientation.
• Bonus: $15,000 Sign-On Bonus, paid quarterly.
• Performance Bonuses: Outcome-based bonus of $1,000 per quarter.
• Estimated Earnings: $77,000 - $100,000+ annually.
