About The Position
**Job Title:** Registered Nurse **Job Specialty:** Home Health **Job Duration:** 13 weeks **Shift:** Day 5x8-Hour (08:00 - 17:00) **Guaranteed Hours:** None **Experience:** Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice experience **License:** Must have license in hand **Certifications:** CPR, BLS **Must-Have:** - Proficiency in HomeCare HomeBase (HCHB) and experience within the last 6 months or completed retraining - Proficiency in OASIS, used in the last 6 months, or completed retraining - Reliable transportation, valid driver's license, and agency-required liability insurance - Willingness to travel up to 75 miles from the assigned care center - Ability to complete all documentation within 24 hours of patient appointments - Acceptance of every patient on the schedule regardless of distance **Job Description:** - Provide, coordinate, and direct the provision of home nursing care according to physicians' orders based on agency policies and procedures. - Supervise and evaluate Home Health Aides and Licensed Practical/Vocational Nurses. - Conduct home visits and attend periodic office meetings. - Manage a caseload of 5-10 patients per day, equating to roughly 6-8 points. - Perform various types of cases, including Start of Care, Wound Care, and Follow-Up Visits. - Open to on-call and weekend rotations as required by staffing. - Cover assigned territory within a specified mileage radius. - Maintain productivity of 30 units minimum weekly. - Adhere to the dress code of wearing scrubs.
