About The Position
**Job Title:** Registered Nurse (RN) **Job Specialty:** Home Health **Job Duration:** 260 Days **Shift:** Day - 5x8 - 09:00AM - 05:00PM - 40 Hours **Guaranteed Hours:** Not specified **Experience:** Minimum two years in a home setting within the last ten years **License:** Current RN Licensure in New York State **Certifications:** Basic Life Support (BLS) required before the start date **Must-Have:** - Recent experience conducting comprehensive assessments of patients - Two years of experience in a home setting, including work for a Certified Home Health Agency, Licensed Home Care Service Agency, or Managed Long-Term Care - Experience conducting assessments similar to UAS such as OASIS - Experience providing primary care to homebound persons - Experience in care management for persons receiving home and community-based services including Hospice and Assisted Living - Home care experience as a paraprofessional or Licensed Practical Nurse caring for a minimum of two patients per week **Job Description:** - Perform initial in-home evaluations of patients, which include assessing health status, strengths, care needs, and preferences to guide the development of individualized long-term care service plans. - Review consumer medical documentation and health referral forms relevant to each case. - Enter evaluation data into electronic forms and transmit as required. - Document concerns, conflicting information, or other issues that surface during the evaluation process. - Emphasize continuity of care, reducing or eliminating fragmentation, duplication, and gaps in the treatment plan. - Discuss with the consumer their healthcare options, support needs, service vendor options, and waiver options. - Initiate communication between the Call Center and eligible consumers for enrollment into a managed care health plan. - Enhance communication and collaborative relationships with interdisciplinary care team members to improve care coordination and facilitate service delivery. - Collect quality review data and other required documentation to support outcome measurements and record case notes into the CRM. - Identify opportunities for health promotion and illness prevention. - Maintain a comprehensive working knowledge of community resources, payor requirements, and network services for the target population. - Meet all standards established for this position as outlined in the corresponding annual performance criteria and bonus template. - Perform other duties as assigned by the Regional Nurse Manager or other project management. Candidates should be ready to conduct hybrid telephonic and in-field assessments. An agreement will be required to ensure no assignments conflict with other UAS assessment roles.
