About The Position
**Job Title:** Registered Nurse **Job Specialty:** Home Health **Job Duration:** 13 weeks **Shift:** Day Shift, 5x8-Hour (08:00 - 17:00) **Guaranteed Hours:** Not guaranteed **Experience:** Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience, depending on the specialty **License:** Must have a valid nursing license in hand **Certifications:** CPR, BLS **Must-Have:** - Proficiency in OASIS and HomeCare HomeBase (HCHB) within the last year **Job Description:** - Provides, coordinates, and directs the provision of home nursing care according to physicians' orders. - Follows agency policies and procedures through the competent application of the nursing process. - Responsible to the Clinical Manager. - Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. - Must be willing to travel up to 75 miles from the home clinic, with mileage reimbursed at the IRS standard rate. - Manages an average caseload of 5-8 patients per day. - Covers a territory within a 60-mile radius of Salem, Oregon, including Marion, Polk, Linn, Yamhill, and Clackamas Counties. - Types of cases include Start of Care, Wound Care, Follow-up Visits, etc. - Required to participate in on-call rotations and potentially some weekend shifts. - Reports for assignment at a designated address in Salem, Oregon. - Expected to travel approximately 200 miles per week. - Achieves a weekly productivity expectation of 30 points. - Attendance at periodic office meetings is required. - Must have reliable transportation, a current driver's license, and agency-required liability insurance.
