RN Hospice Team Leader - The Finders

The RN Hospice Team Leader will be responsible for the day-to-day operations of home-based hospice patient care and management of staff.

Background Profile:

• Accredited School of Nursing Graduate
• Associate’s Degree (Required) | Bachelor’s degree (Preferred)
• 5 or more years Nursing experience & Licensed RN in Arizona
• Your Nursing experience includes Hospice Home-based patient care | Additional Nursing experience in critical, surgical or medical care will be considered
• 2 or more years Team Lead/Supervisory/Management experience (Prior management experience in behavioral health, social work. clinical/surgical/medical will be considered)
• Advanced Clinical Assessment skillset
• You would describe yourself as a Friendly/Positive, Professional, Confident Team-Player who brings an "Always ready to help" attitude
• You are an Excellent Communicator & Passionate about Team Collaboration along with the ability to work cross-functionally with all levels of the organization
• You bring exceptional organizational skills, time management, follow-through, attention to detail & accuracy
• You would describe yourself as a Problem-Solver & Out-of-the-Box thinker good at anticipating needs and driving solutions using your knowledge and creativity
• Proficiency with MS Office Suite including Outlook, Word & Excel
• Must be a US citizen or Green Card holder to be considered
