Duties:
- Implement comprehensive plans of care by providing direct nursing care within a shift for individual patients.
- Initiate interdisciplinary plans of care or guidelines and assist the Patient Care Coordinator in devising comprehensive care plans.
- Evaluate patient responses to implemented care and assist patients in achieving long-term goals through short-term goal setting and education.
- Other duties as assigned.
Job Standards:
- Collaborate with the Patient Care Coordinator and healthcare team to establish interdisciplinary care plans.
- Assess and document health status data accurately.
- Deliver nursing interventions in a timely manner based on assessments to maximize patient outcomes.
- Delegate and coordinate patient care activities with healthcare team members.
- Evaluate and document progress towards established outcomes.
- Address discharge needs and implement appropriate referrals.
- Demonstrate commitment to professional nursing through involvement in clinical practice decisions and performance improvement.
- Support and facilitate changes in clinical practice.
- Utilize therapeutic communication for patient teaching and goal development.
- Assist in the orientation and development of nursing personnel/students as appropriate.
Skills:
- Requires a current Wisconsin Registered Nurse License or a Multi-state Nursing License from a participating state in the NLC.
- American Heart Association (AHA) Basic Life Support (BLS) for the Healthcare Provider or equivalent required within 90 days.
- American Heart Association (AHA) Advanced Cardiac Life Support (ACLS) preferred.
- Strong patient care, staffing, cardiac, and therapeutic communication skills required.
