RN Wound Care
POSITION SUMMARY
Provides patient care to Wound Care Center patients and handles patient intake and discharge processes as assigned and according to the hospital and nursing standards
May also serve as Case Manager to a group of patients as assigned. This entails:. Performing the initial nursing assessment, communicating findings with the assigned physician, assessing the wound stage and measurements, documenting care plans, evaluating the patient's status in regard to desired outcomes, and identifying patient care situations that require intervention. Patient in quality improvement initiatives, providing education to the patient or caregiver, and serving as a patient advocate in the delivery and coordination of Patients' care Performs hyperbaric oxygen therapy (HBO) related duties as assigned, upon completion of the Healogics hyperbarictraining If Healogics hyperbaric training is completed, has the competency updated in employee file? Collaborates as needed with other healthcare providers, Wound Care Center Providers, the Program Director and the Medical Director regarding clinics and patient needs. May function as a Documentation Assistant (scribe) in accordance with Healogics policy.
CLINICAL JUDGMENT/CARE DELIVERY

Clinical judgment is the ongoing process of questioning and evaluating practice and promoting informed practice.
RN Accountability:

Conducts thorough and accurate clinical assessments
Follows Assessment/Reassessment policy
Identifies and prioritizes patient/family needs
Develops, implements and evaluates care and treatment plans. Updates when indicated
Modifies plan to meet established clinical outcomes
Maintains proficiency in equipment operation and troubleshooting in areas of primary assignment
Follows algorithms, decision trees, and protocols
Integrates formal knowledge with clinical events to make decisions
Seeks validation for own clinical decisions
Integrates input from other clinicians for decision-making
Demonstrates the ability to respond to changing workload related to patient census and/or acuity
Coordinates the care/needs of multiple patients, prioritizing patient care to ensure patient needs are met in atimely manner, that care is delivered according to standard of care, and the patient flow is enhanced
Contributes to exceptional patient care by performing actions that ensure compliance with medication reconciliation, CORE measures and other BBGH PI initiatives
Completes required documentation
Functional Pain Scale utilized for assessment of pain on admission, prior to medicating and reassessment of effectiveness of medication
Utilizes gait belt, lifting assistive devices to ensure patient and staff safety when indicated
Recognizes cardiac arrhythmias and intervenes according to standard of care and BBGH policy
Interprets cardiac rhythm strips per BBGH policy
Responds to crises in a calm, efficient manner, directing others concisely
Creates staffing assignments with consideration of staffing mix, skill set and acuity of patients
Provides handoff report at points of transition
Communicates significant patient changes and unresolved issues with Supervisor
Delegates take tasks/care of appropriate staff and ensures timely follow-up
Recognizes and responds appropriately to emergency, Rapid Response or Code situations. Conducts debriefs following critical events
Knowledgeable of and accesses policies on-line
Assists in policy/procedure review and revision
Communicates effectively
Makes suggestions to improve outcomes
Informs peers if standards of practice are not followed
Utilizes BBGH standardized tools and behaviors in conflict resolution
Follows the chain of command for unresolved issues
Supports the implementation of clinical changes and evidence-based practices
If trained/competent in Isolator use, may assist in preparing/providing medications for Pharmacy-after-hours
Participates in performance improvement initiatives
Performs other duties as assigned or requested
Due to changing organizational needs, work hours may vary.
A 30-day notice is required when RN resigns.

EDUCATION
Required: Graduate from an approved Registered Nursing Program
EXPERIENCE/QUALIFICATIONS
Experience:
Minimum of one year recent experience in an acute care setting
License/Certifications:
Required: Nebraska RN license, or license from participating Compact State
BLS for healthcare workers
