Referral Office Coordinator Monday through Friday 8:00am - 5:00pm (varies)
Summary: Utilizing a high degree of accuracy in obtaining and verifying that all registration data; demographic and insurance information, is correct and up to date. Maintain organized, timely support for the multispecialty HQMP Team with communication of referrals between and with patients and insurance companies. Act as a liaison between the HQMP Practice and other community healthcare providers exhibiting and utilizing a high level of customer service skills in all aspects of the job. Responsible for performing a wide variety of clerical duties for administrators and practice staff.
Responsibilities:
1. Referrals
2. Ensure complete and accurate registration, including patient demographic and current insurance information.
3.Assemble information concerning patient's clinical background and referral needs. Per referral guidelines, provide appropriate clinical information to specialist.
4. Contact review with organizations and insurance companies to ensure prior approval requirements are met. Present necessary medical information such as history, diagnosis and prognosis. Provide specific medical information to financial services to maximize reimbursement to the hospital and physicians.
5. Review details and expectations about the referral with patients.
6. Answers patient calls and schedule patient appointments for multiple sites.
7. Facilitates “soft” transfers of patients to the appropriate office for any additional patient needs outside the scope of the HQMP CONNECT centralized referral office.
8. Prepares and process correspondence.
9. Update and maintain phone lists and other tracking lists.
10. Maintaining contact with all HQMP Care Teams, regarding urgency, orders, and scheduling needs of the patients.
11. Be the system navigator and point of contact for patients and families, with patients and families having direct access for asking questions and raising concerns. May assume advocate role on the patient's behalf with the carrier to ensure approval of the necessary supplies/services for the patient in a timely fashion.
12 .Ensures all follow-up calls to patients are performed within 24-72 hours, unless need is urgent or emergent, then prompt action is taken.
13 .Assist patients in problem solving potential issues related to the health care system, financial or social barriers (e.g., request interpreters as appropriate, transportation services or prescription assistance).
14 .Identify and utilize cultural and community resources. Establish and maintain relationships with identified service providers.
15 .Initiate and maintain communication between HQMP and other medical providers from the local medical and specialty medicine community.
16. Research local providers and medical services, establishing new contacts and resources for patients.
17. Act as a liaison between HQMP and other health care providers and facilities.
18. Maintain filing, charting, faxing, mailing and all other clerical functions need for this office.
19. Referral Cycle Maintenance
20. Ensure that referrals are addressed in a timely manner.
21. Maintain ongoing tracking and appropriate documentation on referrals to promote
team awareness and ensure patient safety.
22. Run queries and update flow to improve and track referral progress/completion.
23. Addresses patient complaints in a timely and effective manner; utilizing input from the Business Office Manager, providers and other staff as deemed necessary.
24. Maintains a working knowledge of all aspects of the office; including billing, bills, supplies, charting, workers compensations / no fault, corporate compliance issues and safety issues. Assists in providing directions to offices for patients
25. Other duties as assigned.
JOB QUALIFICATIONS
Education: Required: High School Diploma/GED
Experience:
• 2 years physician office experience, Referral Coordination preferred
• Strong interpersonal, telephone and computer skills
