Job Overview: Registered Nurse Assessor
We are seeking a Registered Nurse Assessor for a hybrid role conducting home assessments and telehealth evaluations. This position requires a blend of in-field and remote work to assess and support patient needs.

Location: Bronx, NY
Compensation: $52/hour
Schedule: Monday to Friday, 3 days home assessments & 2 days telehealth

Responsibilities as the Registered Nurse Assessor:

• Perform initial in-home evaluations and develop care plans.
• Review and document consumer medical information.
• Discuss health care options and supports with consumers.
• Communicate with the Call Center for managed care enrollment.
• Collaborate with interdisciplinary care team members.
• Collect quality review data and maintain accurate records.
• Identify opportunities for health promotion and illness prevention.
• Perform other duties as assigned by management.

Qualifications for the Registered Nurse Assessor:

• 2+ years of recent experience in a home setting (e.g., CHHA, LHCSA, MLTC).
• Experience with assessments similar to UAS.
• Strong organizational, interpersonal, and communication skills.
• Ability to work independently and manage multiple tasks.
• Preferred: Experience with UAS-NY assessments and geriatric care.
• Proficiency with technology and electronic health records.
