Registered Nurse (RN) Weekend Focused Part-Time

Ryze on the Avenue is a community-driven Skilled Nursing and Rehabilitation organization which is located at: 3400 South Indiana Avenue Chicago, Illinois 60616. We specialize in providing a high level of quality care to residents in our behavioral and memory care units throughout the Chicagoland and Suburban communities.    We are looking to fill 3p-11p and 11p-7a shifts at this time.

Essential Job Requirements: 

• Provide direct patient care.   
• Documentation confirming all care provided to patients and residents with in Skilled Acute Care, Long Term Care, Rehabilitation, Assisted Living or Memory Care units as assigned.  
• Provide direction to all Nurse Aides providing direct patient care and assuring that all job duties and responsibilities are successfully completed.  
• Medication Management   
• Providing a high level of customer service to patients, family members and internal employees   
• Additional Duties assigned as needed. 

Requirements

• Current, valid RN licensure in the state of Illinois
• One year of experience in Long Term Care, Rehabilitation, Assisted Living or Memory Care facility preferred.
• Current CPR certification
• Must have excellent communication skills.
