The Home Health Occupational Therapist provides occupational therapy services for Presbyterian SeniorCare Home Health patients in accordance with the individual patient's needs and goals as addressed in the plan of care. The Therapist must have knowledge and understanding of the patient's illness or injury and the ability to assess these needs and plan for appropriate interventions. The role works as part of a joint home health care team including RN, PT, SLP, MSW, and Home Health Aides to assist the patient in achieving optimum outcomes.

What we are looking for:
We need a proven leader who will help our team members and Network leaders as they support the daily living needs of our residents and who desires to develop trusting, caring relationships. The Home Health Occupational Therapist is required to:
- Be a graduate of an Occupational Therapy curriculum that is accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association;
- Have a minimum of two years clinical experience as an Occupational Therapist;
- Possess a valid, current PA license as an Occupational Therapist, or have made application for same;
- Have reliable transportation that is insured to PA State requirements and a valid driver's license;
- Maintain current CPR certification;
Community Health or Home Health experience is preferred.
