Job Description
At Ka’u Hospital, a long-term care facility and Critical Access Hospital, the Registered Professional Nurse IV – Clinical Coordinator/Educator is an essential part of our team.
The Ka’u Hospital Nursing Department provides twenty-four-hour nursing care to all patients and residents at Ka’u Hospital to include Emergency Room, Critical Access Hospital (acute/swing), Long-Term Care and the Adult Day Health Program in accordance with regulatory guidelines, nursing standards and physician instructions.
The Registered Professional Nurse IV is responsible for assisting the Director of Nursing in coordinating and supervising the nursing and clerical staff, and managing the daily operations of the facility by fulfilling the technical and clinical functions, requirements, skills and competencies of the staff. Also serves as a resource in all clinical matters and performs other duties as assigned. The individual must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of patient’s status and interpret the appropriate information needed to identify each patient’s requirements relative to his or her age specific needs, and provide the care needed for all age groups from neonatal thru geriatric.

DUTIES & RESPONSIBILITIES:
Supervision and Coordination
- Assists with the coordination and supervision of the quality of daily care, documentation and planning of the Facility.
- Assures that operational decisions are made based on sound judgment and are compliant with all regulatory requirements, e.g., CMS, OSHA, etc., under the direction of the Director of Nursing.
- Serves as RAI Coordinator
- Supervision
- Provides input into recruitment, and hiring of staff.
- Ensures orientation, and competency assessment of all clinical and support staff including temporary staff are completed per regulatory requirements.
- May perform evaluations as delegated. Supervises and counsels’ staff as necessary. Collaborates with Director of Nursing, to implement improvement plans and monitor the progress of staff.
- Conducts competency assessments for staff as delegated by Director of Nursing.
- Identifies staff development needs, and implements measures to address needs.
- Utilization Management
- Monitor patient/resident criteria for proper level of care.
- Perform Admission, Continued Stay, and Discharge Reviews.
Management of Nursing Care
- Serve as a subject-matter expert and role model in matters pertaining to nursing skills and procedures; teaching skills and equipment; and other clinical and nonclinical areas of expertise.
- May function as the charge nurse of assigned shift.
- Monitors deviations from established standard of care and documentation and takes appropriate action, to ensure quality of care.
- Completes ongoing environmental needs assessments for both personnel and patients, and presents to Director of Nursing.
- Ensures that practices, policies and operations of the CAH/ED and LTC units are current with latest trends, research findings and benchmarks in nursing. This is done through keeping abreast of trends in clinical practice and nursing management.
- Assess educational needs of the Nursing Department staff. Coordinates educational programs and develops plans to address needs.
- Provides ongoing and as needed education to LTC staff and MD’s in completion of LTC assessment and documentation forms to assure regulatory guidelines are met.
- Conduct surveillance activities relating to Infection Control
- Function as a clinical resource for Wound/Ostomy Care Functions
- Participate in discharge planning efforts
REQUIRED QUALIFICATIONS:
EDUCATION: Graduated from an accredited school of nursing. While not required, graduation from an accredited nursing program with a Bachelor’s Degree with emphasis on critical care nursing and/or administration is highly desirable.
Evidence of the appropriate training (e.g. official transcript of diploma) should be submitted with the application to be given credit for education. A legible photocopy will be accepted. However, Hilo Medical Center reserves the right to request an official copy.
EXPERIENCE: One and one-half (1 ½) years of professional nursing work experience. Two (2) years of Long-Term Care experience with the CMS survey process is highly desirable.
Possession of the required number of years of experience will not in itself be accepted as proof of qualification for a position. The applicant's overall experience must have been of such scope and level of responsibility as to conclusively demonstrate that he/she has the ability to perform the duties of the position for which he/she is being considered.
SUPERVISORY aptitude is the demonstration of aptitude or potential for the performance of supervisory duties through successful completion of regular or special assignments which involve some supervisory responsibilities or aspects; by serving as a group or team leader; or in similar work in which opportunities for demonstrating supervisory or administrative capabilities exist; by completion of training courses in supervision accompanied by application of supervisory skills in work assignments; or by favorable appraisals by a supervisor indicating the possession of supervisory potential.
SUBSTITUTION: Possession of a Master’s degree from an accredited college or university school of nursing may be substituted for one year of nursing experience.
CERTIFICATION: Basic Life Support (BLS) in Health Care Provider (adult, child and infant) is required one (1) month after employment.
LICENSURE: State of Hawaii Registered Professional Nurse. Evidence of licensure is required with the application.

Pre-Employment Health Requirements
Offers of employment will be contingent on successfully passing a pre-employment examination, which includes a drug screen and other regulatory medical requirements such as, but not limited to, a tuberculosis (TB) screen. The cost for drug screening and blood draw for QuantiFERON and Titers, shall be borne by the Hawaii Health Systems Corporation.
