Job Summary:

The Care Manager is responsible for developing, assessing, and facilitating complex care management activities for members with primarily physical healthcare needs. This role aims to provide high-quality, cost-effective healthcare outcomes through personalized care plans and education for members and their families.

Responsibilities:

• Care Management Activities: Develop and implement personalized care plans based on comprehensive member assessments.
• Member Outreach: Place calls to members and providers to assess needs, provide education, and monitor progress.
• Home Visits: Conduct home visits as necessary to support member care.
• Assessments & Documentation: Complete assessments and maintain accurate charting to document interactions and care plans.
• Administrative Work: Engage in computer-based tasks, including data entry, when not meeting with members.

Required Qualifications:

• LSW or LMHC licensure.
• Degree from an accredited school of nursing

OR

• Bachelor’s degree in nursing and 2-4 years of related experience in care management or a similar field.

Preferred Qualifications:

• Background in social work, pediatric case management, or mental health counseling (LCSW/LMHC preferred).
• Experience in community or hospital settings, especially working with pediatric populations and community resources.
