Salary/Grade: $15.75 per hour; Grade 75 Non-exempt

Position overview:
The Resident Case Manager will serve in a support role to the Corps Officers, under the direct supervision of the Emergency Housing Supervisor. They will be intimately and integrally involved in the daily operations of the Norristown Family Residence, understanding the need to provide support from a trauma-informed perspective.
The ideal candidate will remain teachable and interested in their ongoing professional development.
Responsibilities:Description of Position:
Intake: In conjunction with the Lead Monitor, the RCM will handle all aspects of the intake process for the Norristown Family Residence, including but not limited to the waiting list, screening process, eligibility requirements, documentation collections and program entry.
Assessment: The RCM will assess potential clients for each housing program using assessment forms and Adult Service Plans, which include short- and long-term goals with each client. The goals of the plan will promote self-sufficiency and self-determination for each client and will be tailored for each individual client.
Ongoing Case Management: The RCM will meet regularly, at least weekly, with each client and will provide support and follow-up where necessary in the areas of employment, education, budgeting, counseling, life skills, parenting skills, housing, income, and other areas as written into clients’ care plan. An ongoing written narrative will be kept on all casework activity and all information will additionally be entered and updated into the Clarity database system.
Client Education: As directed by the Emergency Housing Supervisor, the RCM is required to work in conjunction with the other Housing Resource personnel and interns to provide educational classes for clients in the Family Residence. Topics can range from budgeting to life skills, domestic violence, etc. and will be selected based upon results of a Needs Based Assessment.
Familiarization with Community Resources: The RCM will develop an in-depth knowledge of extra-institutional supports to provide suitable and informative services to each client. The RCM is required to become familiar with outside programs, utility assistance programs and mainstream resources to better assist the program participant.
Empower participants to meet program goals: The RCM is responsible for ensuring that clients can successfully meet program goals of obtaining permanent housing, increasing skills and income, and achieving greater self-determination.
Reporting documentation: The RCM is solely responsible for producing, maintaining and gathering appropriate documentation for each program participant. Additionally, the RCM will maintain organized and thorough case files, collect, and report out on vital statistics and program developments, as well as keep in-depth cases notes for each client.
Other:
- Serve on Resident Case Review Team and attend monthly meetings
- Serve as a Level 1 Reviewer for Client Grievances relative to Case Management
- Assume some clerical duties, including answering phones, occasional monitor duties and other tasks as assigned by the Emergency Housing Supervisor and/or Corps Officers.
- The RCM will be required to read, understand, and follow the local operations manuals and perform duties as assigned, including supporting the operations of the Family Residence.
- Involvement in the total Corps program with willingness to help in all areas as needed, including dining room, outdoor/indoor activities, children with problems, daily assembly, etc.
- Promote observance of the moral and ethical codes affecting the Norristown Corps
- Attend and contribute to Department & General Staff Meetings.
- The RCM will be required to work additional hours during the operations of the CODE Blue & Winter Shelters to provide any requested case management for participants.
Requirements: High School Diploma or equivalent; Bachelor’s Degree preferred. Excellent people and problem-solving skills. Must have access to and use of an insured & inspected vehicle in good operating condition. As a Mandated Reporter, must provide proof of having completed training on recognizing and reporting child abuse meeting the training standards of PA Act 153 of 2014 within 30 days of employment with recurrent training required every five years thereafter. Child Protection Clearances PA Act 153 requires that all prospective employees complete the following clearance and background checks: PA Child Abuse History Clearance, PA State Police Criminal Record Check and FBI Criminal Record/Fingerprint Check. Must have an understanding of and support for the Mission of The Salvation Army.
