We believe everyone recovering from mental health issues or substance abuse is able to work at a good job - if the right job and work environment can be found for them.Are you ready to make a Difference?Highland Rivers Behavioral Health is seeking a SSI/SSDI Outreach, Access, and Recovery (SOAR) specialist to serve individuals in Cobb County. The SOAR specialist assists eligible individuals with the application process for SSI/SSDI. Duties and Responsibilities include:

• Provision of case management services with regards to linkage to and coordination with Social Security Administration and assistance with disability application purposes.
• Manage a caseload by ensuring all services are authorized, chart paperwork is complete and current, and individual charts are closed according to Policy and Procedure.
• Completion of disability application and submit to SSA within 30 days of referral
• Completion of SSA Functional Report and all SSA paperwork with Individual.
• Completion of Medical summary report for all applications and submit to Disability Determination Services within 60 days of referral.
• Coordination with SSA, ODAR, Disability Adjudication Services and advocate for individual benefits
• Provision of representative services to Individuals served including representation at the hearing level.
• Coordination with other service providers within the agency by attending treatment team meetings and complete clinical staffing to indicate updates on cases of Individuals served.
• Other duties as assigned.
