Salary: $45,000 to $50,000
SUMMARY OVERVIEW
The Senior Care Manager must possess advanced Case Management skills. S/he will serve as a resource, mentor and trainer to other care managers. S/he is responsible for both identifying issues important to the effective functioning of the team and discussing them with the Care Management leadership. When new programs and policies are adopted, the Senior Care Manager will assist Care Management leadership with implementation.
ESSENTIAL FUNCTIONS
Essential Job functions: Duties _______ through _______are designated as ADA Essential Functions and must be performed in this job. All other job duties are secondary functions.
- Assume/maintain primary responsibilities of a Home Care Case Manager
- Serve as a resource, mentor and trainer to the Home Care Team.
- In conjunction with Care Management leadership, assist in orientation of new staff as well as training of Home Care Case Managers on new/revised policies and procedures.
- On-board and conduct trainings for all new hires in accordance with New Employee Orientation.
- Shadow new CMs during introductory period both in-person (and telephonically, if applicable),
- QI and Peer Review of records and documentation for new Case Managers,
- Report on all progress made by new Case Manager to Team Manager,
- Act as a liaison between new Case Manager and Team Manager,
- Provide ongoing support and guidance for new & existing CMs (Home Care and Admissions),
- Participate in job interviews for new CM applicants and provide recommendations to Home Care leadership,
- Attend outside trainings and present information to Team Managers/Home Care department.
- Take the lead on Consumer Directed Care/Tempus related questions. Attend meetings as needed.
- Perform other duties as assigned by Care Management leadership.

SUPERVISORY RESPONSIBILITY
This position has no supervisory responsibilities.
WORK ENVIRONMENT
This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, printers, copiers, filing cabinets.

POSITION TYPE/EXPECTED HOURS OF WORK
This is a full-time position, Monday-Friday, 9:00 AM 5:00 PM.
TRAVEL
This position required up to 25% travel. Must be able to travel to the community to perform visits at consumers' homes (may have limited accessibility), and travel under all weather conditions.
REQUIRED/PREFERRED EDUCATION AND EXPERIENCE
- Bachelor's Degree is required
- Two years' experience in elder care case management.
- Licensed Social Worker and relevant work experience preferred, but not required.
