SERVICE COORDINATION
This individual will provide service coordination throughout Washington County for families with children who are eligible for Early Intervention under PA Act 212. Work is performed with a considerable degree of initiative and independent judgment by established regulations, policies, and procedures. The position reports directly to the Team Supervisor who provides general guidance around program issues and evaluates the effectiveness of the Service Coordinator’s activities.
II. RESPONSIBILITIES
- Provide explanation of Early Intervention Services to families in Allegheny County.Coordinate and schedule screenings and assessments.
- Identify and coordinate community resources and services beneficial to child and family.
- Coordinate with medical and health providers.
- Monitor child and family needs through use of appropriate instruments and family interviews.
- Provide parent education and support.
- Facilitate and participate in the development and review of the Service Coordination Plan and the Individualized Family Service Plan.
- Monitor delivery of Early Intervention Services through regular contact with family and communication with service providers.
- Explain parent’s rights and procedural safeguards.
- Initiate the development of a transition plan to pre-school services.
- Conduct and attend meetings as necessary.
- Attend and participate in team meetings, staff meetings, internal and external training and conferences in order to meet required 24 hours of annual training.
- Maintain expected level of productivity.
- Complete all necessary documentation and submission of paperwork within agency timelines.
- Adhere to agency’s requirement for office attendance during and after training period.
III. QUALIFICATIONS
A. PROFESSIONAL KNOWLEDGE AND ABILITIES
- Demonstrate knowledge and understanding of children with developmental delays and conditions that put a child “at risk”.
- Demonstrate knowledge of child development.
- Knowledge of the laws and regulations governing the Early Intervention Program in PA.
- Ability to administer and interpret child and family assessment instruments.
- Ability to maintain records and documents in an accurate and orderly manner according to agency policies and procedures.
- Ability to establish and maintain effective working relationships with families, staff, physicians and other community service agencies.
- Ability to communicate effectively, both orally and in writing.
- Ability to manage email, navigate the internet and effectively utilize a web-based data system to manage caseload information.
- Possess adequate computer and typing skills. \
B. JOB REQUIREMENTS
1. Provide Service Coordination services to families throughout Washington County.
2. Possess a reliable motor vehicle and a valid motor vehicle license to travel throughout Washington County.
3. Have Acts 33, 34 & FBI clearances.4. Be able to work at floor level with young children.
5. Be able to lift and handle young children.
C. EDUCATION AND EXPERIENCE
Bachelor’s Degree in early intervention, early childhood special education, early childhood education, child development, family counseling, family studies, psychology or other comparable social science field and one year of related experience
OR
Associate’s Degree in early intervention, early childhood special education, early childhood education, child development, family counseling, family studies, psychology or other comparable social science field and three years of related experience
