- Recruit patients from Wards 5, 7, and 8 to receive sexual health care services
- Assist patients in setting short and long tern sexual health goals with emphasis on social needs, barriers to care, pregnancy planning and understanding contraception.
- Coordinates and facilitates group education
- Ensures patients complete referrals and treatment through reminder calls, assistance in making appointments, transportation assistance, etc.
- Responsible for meeting and maintaining monthly departmental targets for new patient enrollment and ongoing follow-up and support.
- Provides referrals to community resources when necessary (i.e. housing, job training programs, and mental health supports, etc.)
- Assists with generating reports which may include analysis of patient population, updating excel spreadsheets, efficacy of education, group requiring follow up, UDS reporting, etc.
- Documents patient support activity, care plans, and assessments in Healthy Start database and electronic medical records.
Minimum Qualifications:
- B.A. or equivalent professional experience required
- At least of two years of experience in the public health related field.
- Bilingual in Spanish highly preferred
- Commitment to work with underserved populations and ability to work in nonprofit environments.
- Demonstrated cultural competence in communicating with low income , African American populations
- Experience with education related to reproductive health or other health topics required. Experience supporting at risk populations preferred.
- Strong written and verbal communication skills.
- Ability to travel between other COH locations is required
- Ability to work evening and weekends. Flexibility is a must for this position.
- Ability to work with computers and electronic health records.
- On-site: A majority or all of the position is required to work on-site. Please note that remote work designations are subject to change or fluctuate at any point in time and the supervisor may require in person learning for a specific amount of time after hire.

About Community of HOPE:
Community of Hope is a mission-driven, innovative, and rapidly growing nonprofit. For nearly 40 years, we have provided healthcare, housing, and supportive services for low-income, underserved, and homeless people in Washington DC. As a Federally Qualified Health Center, we provide medical, dental, behavioral health, and care coordination services for the whole family at three locations in DC. Community of Hope also has a strong emphasis on maternal and child health, with a midwifery practice and the only free-standing birth center in DC. In 2020, Community of Hope provided about 28,400 medical visits, 7,000 dental visits, and 10,000 behavioral health visits for about 11,000 patients. Community of Hope is also providing community walk-in COVID testing and COVID vaccines. Community of Hope is also one of the largest providers in DC of housing and support services for families experiencing homelessness. Through providing these programs, we live out our mission to improve health and end family homelessness to make Washington, DC more equitable.
