Position Summary:
The primary purpose of your job position is to assist in planning, organizing, implementing, evaluating, and directing the overall operation of our facility’s Social Services Department in accordance with current federal, state, and local standards, guidelines and regulations, our established policies and procedures, and as may be directed by the Director of
Social Services and/or Administrator, to assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis
Responsibilities:
- Assisting Director of Social Services
- Making phone calls to set up transportation or doctor appointments for residents
- Transporting residents to doctors' appointments
- Must be able to assist social worker(s) in supporting patients and families through education, financial planning assistance, liaison with community agencies, etc.
- Providing residents and family Medicare/Medicaid and financial information when requested
- Maintaining accurate social service progress notes
- Updating the social services directory
- Obtaining information from community social health and welfare agencies in order to meet the resident’s needs
- Planning and organizing time and duties to efficiently maintain caseload
- Protecting each resident’s file and data with confidentially per HIPAA standards
- Must perform proficiently in all competency areas including but not limited to: patient rights, and safety and sanitation
Qualifications:
- Must be a high school graduate or equivalent
- Previous experience in skilled nursing preferred
- Must be able to read, write, speak, and understand the English language
- Strong communication skills
- Ability to multitask in a busy setting
- CNA certification preferred
- Valid California Driver's license that is in good standing
